Please enter your name exactly as mentioned in your Canadian ID proof. If you have a middle name in your Canadian ID proof, you may enter your middle name either in the first name field or in the last name field.
Please note that space is not allowed in the first name field and the last name field and hence to enter your middle name, just write it together either with your first name or with your last name in the designated fields.
